Last year I sent out a Apologia Science book media mail. I didn't insure it as the buyer didn't specify that she wanted it. I should have though because the cost of the book isn't cheap. Anyway, I happen to have had my receipt still to prove that I had actually sent the book out. Had we insured it we could have been reimburse by the post office, but we didn't.

Also...using Paypal is the way to go. As of right now, I am waiting for a refund from Paypal because I purchased material from someone that never sent anything out to me or 12 other buyers. You are insured when you pay through Paypal, but you must file a claim before 45 days of payment date.

As far as repayment, I think if you have your receipt that it was sent out and can scan and send the buyer a copy of it for proof of delivery...you aren't at fault and need not repay her. But if you feel you want to repay her, you can meet her halfway and repay her half of what she sent you. If you don't have proof that it was sent...I would repay her the full amount.

Media mail supposedly takes 2-9 days, but also depends on the distance of the delivery. I think sometimes it can take up to 14 days. After that I would start to worry.

I just started sending my orders out media mail with delivery confirmation. It is an extra .75, but you have a tracking number and can keep an eye on its whereabouts. Peace of mind! Also you can give your buyer the tracking number so that she can watch and know it's on its way.

Also if you go through Paypal you can send it through their postage thing. Print the label, box it up, and the mailman picks it up at your house. I haven't used it yet, although I have had sellers send things to me that way and I really like it, because I can track my package.
