I do all of the above and then add a tiny bit of guar gum. I use about 1/6-1/5 a tsp for about 1.5 liters of milk. Just strain and then vigorously whisk in the guar gum and you'll have smooth milk in no time. You should be able to find it in the baking aisles of larger grocery stores.
