Cedar, It sounds like you have handled it all very well. I think you should follow the woman's advice and not sign the review if it is the same. I also think you should make a bulleted list of what you have done and for whom (similar to what you wrote) so that the facts are in black and white for him and for anyone else who is above him. The list of all the times you went above and beyond that he acknowledged is good justification by itself for "above and beyond." I'm glad the woman is going to bat for you since your boss is not even really looking at what you turn in to him. Paying fair wages is important for any business which wants to prosper.
