Thin the yogurt, if necessary, to the consistency of milk. Put into a jar. Add the grains, cover open top of jar with a paper towel and a rubber band, and let sit on the countertop for at least 8 hours. Remove the grains from the yogurt, and put them into fresh milk. Let stand on counter (covered with a paper towel and a rubber band) for 8 - 20 hours, or until the kefired milk begins to separate somewhat. That should do it. For unusually unhealthy grains, you can repeat the yogurt soaking. This is a great way to "freshen up" kefir grains every now and then; maybe once a month.
