Kefir grains can be kept almost indefinitely,
unless they were horribly abused.

Take them out of the fridge, rinse well,
and put them in your usual kefirable substance
(water, milk, or whatever you use).

It may take a few batches to get them back to
their previous performance, but keep trying.
