I can't speak to your friend's health issues, but most folks find they can consume a greater number of calories and suffer fewer digestive issues when they eat 2 or 3 large meals per day as compared to when they eat 6 smaller meals. Not knowing your friend's fitness choices, or most anything else disables any ability to comment, but I agree that unless the low blood sugar was diagnosed medically, likely there is something else going on that is mistakenly being called low blood sugar. I've tested dozens of people who came to me with self-diagnosed low blood sugar only to find that their sugar was normal.
If your friend wants to gain fat, eat more. To gain muscle, train like a body builder, and eat more.
