Definitely, I assess each wound on an individual basis, and make decisions on the spot.
Wounds that bleed often need less cleaning. Wounds with dirt ground in to them deserve cleaning. Wounds obtained on farmland around animals deserve special consideration.

The same answer for fever. The overall health of the individual must be taken into consideration. Individual answers are appropriate. Children can generally handle fever better than adults, but I wouldn't want to risk a person's life just over my philosophy.
