If there is to be any concerted effort at transition, I would not expect almost anyone to require more than 48 hours or so to complete the move to 811. Sure, people take longer, sometimes, but that is a choice, not a physiological necessity.
Most people, even the ones you mention, could simply go direct to 811 while expecting their health to greatly improve, even during that short 48 hours of physiological transition.
