In general it would/should be worse with starch. But it also may depend on what fruit(s) are consumed. That is, their phytonutrient profile, like phenols/antioxidative capacity etc.
For example, it has been found that orange juice, quote: "We have also observed that the intake of orange juice along with a high-fat, high-carb, meal prevents the increase in LPS, TLR-4, TLR-2, ROS generation, and other inflammatory responses in mononuclear cells and in plasma."
