Marie, if you like myself are uk, I think it may simply be that the British consider the term "Doctor", plus a name, to mean a medical doctor only. When one has gained a doctorate other than specifically to practice medicine, we qualify it by the term "Doctor of ---" (their discipline). In the States, they seem ok to call anyone with a doctorate just that, without explanation - as Dr Graham does. Just an impression; correct me if I'm wrong anyone!
