Yes, it is necessary for a tooth to be decayed for an
abcess to develop, they do not "spread" to adjacent
teeth.

Drainage of abcesses is not automatic at all; you are
fortunate that your body has been able to deal with
the infection in this way. Abcesses can a)
encapsulate, ie remain dormant but can cause problems
again, b)cause such massive pain and debility as to
require removal, c)worst-case scenario if left
untreated, eat into neighbouring jawbone causing
constant pain, leading to death by septicaemia,
d)drain themselves as yours is doing, to a partial or
total extent.

Nevertheless, if yours still drains after a year or
more, I suggest you are best having it out.
