Most refuse to give up one or more bad habits:
1) refusing to bring down their fat levels
2) trying to get enough calories on greens
3) still eating cooked foods
4) still hooked on coffee/tea/salt etc.
5) not incorporating enough positive lifestyle factors: exercise, sleep, sun, positive emotional poise etc.
6) still looking/trying for quick fixes or "remedies" instead of long term health development. These "remedies" end up causing more damage-but they blame the diet.
7) giving up. You only fail when you stop trying

There are always proximate as well as distant causes for both health and disease.
