Here's what could be going on:

1. You're overloading the PC. 4 qt is not very big for beans;

2. Your gasket isn't sealing correctly;

3. You are leaving the heat up too high when it comes to pressure;

4. Your pot is defective

The whole point of pressure cooking is keeping the steam and heat INSIDE the pot. What you're experiencing is totally off.
