Here's a good one. My guess would be that they had too much in it, and, of course, the pressure relief valve failed. I'm sure they had pork in there as well, but I'm not sure that that would have contributed to the explosion. Presto recommends adding some oil to keep foaming to a minimum. Hope this doesn't scare anybody away from pressure cooking. I didn't have to happen.
