I'm a nanny, not a parent, but I have some problems with eczema
myself. The best fix for me, better than hydrocortizone or
antihistamines, has been olive oil rubbed into the affected area. You
might also try switching to a fragrance and chemical free laundry
detergent if you haven't already, and put an extra rinse cycle in the
wash.

If it's the citrus, I would expect the first symptom to be diaper rash, or
just redness/irritation around the anus. I was not under the
impression that citrus was an actual allergy, it can just be too acidic
for baby's still delicate skin and digestion. I'd cut it (and strawberries
too) until baby is a year old.
