How does one know if she has heart disease? Will one's doctor necessarily have said, "You have heart disease"? Is it possible for someone who has not had a heart attack to have it? (I'm assuming anyone who has had a heart attack has heart disease.)

What conditions short of a heart attack are considered heart disease, if any?
