I had the same problem with C&S and a saw a nutritionist who recommended a few things that worked:
1. For one whole day, just one day, everything that goes in your mouth stays in, and swallow it. You can't gain actual weight in a day, so just do it for one.
Then try again the next day. Really use your willpower and make yourself swallow whatever you put in your mouth.

2. This helps too- take smaller bites- it's harder to spit things if there's not much there to begin with, and you don't feel so bad about swallowing it.

3. Before I ate, I would say to myself: "I want to take good care of myself" 5 times. Just 5, and it takes 10 seconds. This helped me calm down, choose better foods, and then really eat them.
