There are a few. The Anointing of the Sick is the sacrament for the sick, and it can be done within the context of a Mass that could be held in your grandfather's room if he isn't able to get out. Family and friends can be present and you cna make it a really special time. You can do it while the sick person is still strong and lucid enough to enjoy the ritual. Viaticum is the ritual that is for people for whom death is imminent. It means "food for the journey" and it is meant to be the person's last Holy Communion.
