Having seen an untreated case of mastitis, not pretty. The infection can quickly become systemic and kill the dog. The one I saw was "lucky" the infection literally exploded her teat in five places ... if it hadn't blown up the vet said she would have died. I never want to see the inside of a dog's teat again.
strongly encourage the vet please!
