Very often when a heart problem manifests a dog who has been an indoor dog may want to be "outside" because they feel like they are getting more fresh air that way. It's not really 'true' but the wide-open space is something they think they need.
VET ... absolutely. Outdoor dogs aren't safe -- particularly not one the size of an eskie -- it's too much a target for predatory birds like eagles, hawks, etc.
But the fact that she is now shunning what used to be a cozy *enclosed* place like under the bed for OUTside?? VET ... for sure. That's got to be your first step.
