Sorry for the late reply, maybe you have solved your problem...
One thing that could cause this behaviour is if you recently changed brands of cat litter. Cats often get picky and grow accustom to a certain type, texture of litter and will ablsolutely refuse to go in the box in another type of litter.
If you havnt changed types, maybe look at trying another kind, provide another box with a different type and see if the behaviour changes.
Specially if the vet checked and there are no problems health wise.
